Healthy Heart Ambassador
The Healthy Heart Ambassador (HHA) Program is a 4-month Centers for Disease Control and Prevention (CDC) recognized initiative designed to help adults with high blood pressure—or those taking medication for high blood pressure—manage their blood pressure through proper home monitoring, lifestyle changes, and individualized coaching. Participants receive personalized consultations twice a month with a trained Healthy Heart Ambassador. They will learn how to accurately measure their blood pressure at home and attend monthly nutrition and cooking seminars. Programs can be held virtually or in person.

Meet with a facilitator twice a month for blood pressure checks, weekly support check-ins by phone, email, or text, and monthly nutrition education seminars

One on one meetings and check-ins can be in-person, virtual, or by phone

Nutrition seminars can be online or in-person. We do encourage participants to attend in-person nutrition seminars if available

Access to a computer and cell phone is important for virtual participants
Participants

Must be at least 18 years old or older
-
Told they have high blood pressure and/or on medication for high blood pressure
-
No cardiac events within the last 12 months
-
Does not have atrial fibrillation, arrythmias, and have, or be at risk for, lymphedema
